If you would like to have access to your information, please stop by our switchboard or any reception area, Monday through Friday 8 a.m. to 5 p.m. to request your PIN number and letter.  To protect your privacy, we will not give PIN numbers over the telephone or email.  If you are a new patient please call our office to schedule an appointment.
